If you have scheduled a first-time appointment with HTS & Co., we hope you will take a few minutes to complete our Client Intake & Release Form.  The information we ask of you helps us have a better idea of how we can best meet your needs. You may email the completed form back to us, print it out and bring it in at the time of your appointment, or simply complete the forms upon your arrival at Heal Thyself and Co.

HTS & Co. accepts Visa, Mastercard, Discover, American Express credit cards, checks, and cash. And if you are insured by a provider that covers massage therapy, we may accept payment  by health savings accounts (HSAs) and flex accounts.
